Educator Plan/Self-Assessment

  • 1) Propose goals and complete Self-Assessment 

    • At least one SMART goal directly related to improving the Educator's own Professional practice
    • At least one SMART goal directly related to improving student learning
    • Consider shared group goals (e.g. grade level, subject-area, department teams, or other groups of Educators)

    2) Evaluator reviews goals 

    3) Educator and Evaluator sign the Educator Plan 

Evidence Collection

  • Evidence related to one or more Performance Standards may include, for example:

    • Fulfillment of professional responsibilities and growth (e.g. self-assessments, peer collaboration, professional development linked to goals in Educator Plans, contributions to school community and professional culture)
    • Active outreach to and engagement with families
    • Progress toward attaining professional practice goal(s) and toward attaining student learning outcomes/goal(s)
    • Any other relevant evidence from a source that is identified and shared with the educator
